About

Karen Dorsey Sheares, Principal

I am a physician, epidemiologist, and health systems leader working at the intersection of healthcare quality, research, policy, and technology.

My career has been shaped by working inside institutions responsible for defining, measuring, and enforcing healthcare quality. I have spent years navigating the space between clinical practice, research, policy, health technology, and measurement, often where the limits of formal frameworks become visible, and where judgment, interpretation, and systems awareness matter as much as technical expertise.

Across my career, I have worked inside academic medicine, national quality organizations, and federally funded evaluation and improvement efforts. This broad experience has given me a practical understanding of how incentives, accountability, and organizational structure shape what is possible.

I advise leaders with respect for rigor, attention to systems, and a clear-eyed understanding of how healthcare institutions actually function.

How I Work

I work with leaders who value depth, clarity, and integrity who eager to engage complexity rather than bypass it.

My advisory approach is:

  • Context-driven rather than formulaic

  • Grounded in evidence, systems thinking, and lived institutional experience

  • Attentive to constraints, power, incentives, and unintended consequences

  • Selective by design

I am often brought in when organizations need a trusted external perspective to help translate ambition into strategy, align data with decision-making, or modernize thoughtfully without losing what already works.

Professional Throughline

Across my career, I have sought out work where evidence carries consequence, and where analytic rigor must support decisions that affect people, systems, and public trust.

I bring the greatest value when the right answer is not simply technical, and where careful judgment, historical awareness, and respect for lived impact matter as much as methodological strength.

In practice, this means I approach advisory work with a bias toward clarity rather than certainty: helping leaders surface assumptions, understand constraints, and make decisions that are intellectually honest, contextually grounded, and durable when implemented.
